Instructions

• Select the weekend you are attending from the dropdown list.

• Enter your full name in the "Add your Name" box.

• Under Personal Photo Choice, please check one of the radio buttons.

  • If you are uploading a new photo on this page, select "Use uploaded photo"

  • If you prefer to use a photo previously provided, check the "Use Previous Photo" checkbox

  • If you would like to decline to have your photo in the retreat booklet, check the "Decline Photo" checkbox

•  Click on the Upload Personal Photo button, and select the .jpg or .png file you would like to represent you in the retreat booklet. (this is the only item on this page that is optional)

• Click Submit
